UMB approval of an application or proposal prior to submission is required, using the ORD Routing Form. The routing form database is hosted on a secure server, with all data communication encrypted for maximum safety. From the log-in page, you may log-in or apply for access (that is, obtain a user name and password). Using a Principal Investigator log-in, you may: - Create new routing form

If you are authorized with Administrator privileges for a defined unit (division, department, center, etc.), your log-in allows you to: Note that certain sections of the routing form, particularly the conflict of interest questions and questions related to intellectual property, should only be completed by the PI or with direct input from the PI. Complete the routing form on-line. Once the data are entered, click on the “Submit” button and a PDF is automatically generated and emailed to the PI and Administrator. Print the PDF form, attach the application or other project documents, and route for signatures in the usual manner.
